In spite of this growing buzz, however, virtual care at our institution and most other academic medical centers remains more of a future vision than a daily reality. For smaller or community-based providers, this gap is even wider. For virtual care to move from an innovation pilot to a standard service, five questions need to be answered by any provider: Which clinical services should be offered virtually, and why? Which technology tools will meet the demographic, clinical, and business needs for these services? Should telehealth programs be offered directly to patients or only among providers? How does virtual care create value for my practice or organization? How can this value be assessed from the patients perspective as well as the organizations? Our strategy has been guided by nuanced answers to these questions. Here is what we did at BWH, organized by the three clinical and strategic problems we aimed to solve through virtual care. Virtual Visits: Improving Access and Population Health in Chronic Disease Management Our virtual care strategy began with video-based visits for outpatients with chronic diseases. In 2015 we started to engage those clinical departments whose providers saw patients with conditions requiring frequent follow-up visits and infrequent physical exams, and who lived in Massachusetts but had difficulty coming into the office. While a broad set of patients met these conditions, our providers targeted those with inflammatory bowel disease, diabetes during pregnancy, mood disorders, hypertension, ischemic heart disease, prostate disease, and airway disorders. We outfitted exam rooms and provider offices with cameras and trained the providers how to use a secure video platform to connect with patients remotely, while designing practice workflows to effectively move patients through the program. Providers were compensated at a rate roughly commensurate with routine office-based billing, regardless of payer mix. In addition to working with participating providers to target clinical populations for whom virtual care would be appropriate, we asked providers to offer virtual visits to align with BWH population health and access goals. We specifically focused on patients who would not need to come into the office for in-person care in addition to receiving virtual care, so as to avoid increasing overall utilization. And we chose providers willing to open up their schedules to see patients virtually, either more efficiently during their clinical hours or during their non-clinical time. Insight Center Approximately 600 visits have been conducted virtually through this program to date, freeing up about 200 hours for participating providers to see patients. Among patients surveyed after their initial encounter, 97% were satisfied with the experience and would recommend the program, and 74% felt that the interaction actually improved their relationship with their provider, allaying some of our concerns that what patients would gain in convenience they would lose in a remote interaction. From a population health perspective, where our goal was to remove some of the lower-value in-person visits to make room in our providers schedules for more-complicated cases, we were encouraged to find that 87% of patients said they would have needed to come into the office to see a provider face to face if it werent for their virtual visit. The greatest adoption of this program to date has been in our medical and surgical specialties, as well as in psychiatry. As we grow this program, we plan to evaluate its value in a number of innovative ways. One hypothesis is that we can improve no-show rates by offering virtual care, given the dramatic improvement in patient convenience. Related, we will be tracking different measures of the time saved by virtual care for both providers and patients. After an initial pilot of 700 e-visits among a subset of our primary care practices, we began expanding across our system. Unlike video-based visits, which take less total time for the patient (because there is no transit or practice wait time) but about the same time for the provider, we expect provider and practice efficiency gains for e-visits to be significant. The efficiency metrics we will assess from our medical record platform (EMR) are still in development, but from early experiences we know that a 15-minute office visit for urinary symptoms or cough can be done in under five minutes through this program. We also plan to experiment with midlevel provider models, where dedicated physician assistants, for instance, would spend part of their time each day answering e-visits, becoming increasingly proficient and responsive to the growing number of patients using theservice. As we and other systems consider payment models thatincentivize population-based, panel-driven provider work and compensation, we will likely test approaches where provider time is strategically protected for asynchronous care for large groups of patients. We believe this program will become increasingly vital as the market offers more virtual urgent care options. E-Consults: Broadening the Reach of Specialty Care In addition to telehealth services offered directly to patients, we use virtual tools to improve care and communication among our providers. Our E-Consult program attempts to solve a daily problem in ambulatory care: Aclinical question arises thatmay or may not require a referral but thatthe primary clinician cannot answer alone. These questions are often related to referrals (Does this patient need to be seen by you, by a colleague, or can I manage with your support?) or other moments of clinical management (Im choosing between two medications, can you help me select which is more appropriate?) thatface primary and specialty care providers on a daily basis. E-consults ordered in our EMR provide a dedicated, responsive, and findable communication channel thatcan be ordered like any medication or test. Providers formulate a question and synthesize relevant information, and then route these requests to established specialist teams we centrally organize, support, and compensate at a rate similar to e-visits. In the past year of the program, roughly 300 of our providers have sent nearly 2,000 questions to our specialist e-consultants. Primary care physicians represent 80% of the sending volume, with specialist senders making up the remaining 20%. Among our 20 specialist teams, the highest-volume groups consistently are cardiology, endocrinology, gastroenterology, hematology, infectious disease, orthopedics, and urology. The programhas two strategic aims, targeted atimproving access and population health: to better support primary care providers so that more primary care can be managed without referrals, and to assure that when referrals do take place, they are better packaged and triaged by the referring provider so that the specialty consultation is more effective for the patient. In a chart review last year of a large sample of these questions, we found that when a sending physician intended to refer a patient to a specialist but first virtually consulted a specialist in that field, about 50% of the time the referral was avoided. From a cost perspective, this means fewer unnecessary specialist visits a significant driver of medical spend. From a specialty access perspective, these results suggest that reduced referrals generated greater capacity for complex and appropriate specialty care. We have also received consistent feedback that e-consults are deeply helpful and supportive to primary care providers and fill the management gap created by challenging clinical scenarios. You'd have to slog through miles on the treadmill or the elliptical, alone. It was daunting. Sure, there was the option to take a group exercise class, but unless you were step aerobics acolyte, you probably steered clear of that room. Now, group exercise is all the rage; it's redefined fitness as we know it. Zumba CEO Alberto Perlman chalks it all up to the rise of the instructor. Consumers are increasingly willing to pay for an expert to show them how to exercise. "Youre seeing all these trends from boutiques to Zumba to CrossFit, and everything, because the fitness industry realized that it was about the instructor and it was theconsumers who told the fitness industry that." The rise of the instructor, he says, is "the underlying trend behind everything that is happening." As consumers turn to boutique gyms, it could pose a threat to traditional, bare-bones athletic facilities. Star instructors have been the center point of branding business empires and at-home fitness from Jane Fonda andTracy Anderson to Billy Blanks inTae Bo videos from the late '90s but that trend has been ostensiblybeen moving from television screens to people's gyms. Think about it: If you're a spinning fan, youknow which classes you like to go to maybe that teacher has the best playlist or says the right platitudes. There's probably an instructor who just yells at you and makes you hate working out. Unsurprisingly, higher-end gyms like Equinox tend to get top-notch instructors who make the workout about the people in the class, as opposed to people who are just looking to get paid to get a workout in. Popular indoor-cycling studio Flywheel always places an emphasis on hiring top instructors. She said the company looks for people who are motivational, enthusiastic, confident, and most importantly, come equipped with "athletic prowess." "SoulCycle is open about the fact that the company recruits performers as much as, if not more than, people with a fitness background," New York Magazine reported in 2013. Master trainer Janet Fitzgerald even told the magazine that she "would rather not take an outdoor triathlete" as opposed to a dancer or SoulCycle enthusiast. While that could make for an entertaining class, expertssay it doesn't lead to a safe class. "These instructors know little, if anything, about physiology, biomechanics, or real cycling technique. This is also a sad expos of this industryand not just at this studio," master Spinning instructor Jennifer Sage wrote for the Indoor Cycling Association, referring to SoulCycle, but also shedding light on other instructors in the cycling industry. Such legislation has been introduced in the New Jersey General Assembly, which is the lower house of the states Congress. If successful, residents will be able to get a medical marijuana prescription to alleviate the ails that come with monthly menstruation. [embedded content] The bill was introduced by three representatives, Tim Eustace, L. Grace Spencer and Angelica Jimenez all of whom are Democrats in a majority Democratic Congress. Tim, the only male to author the proposed marijuana for menstruation law, said that the current policy against use for menstrual cramps was both unfair to both women and men, reported local paper Hoboken Patch. By denying women in New Jersey access to a means of treating dysmenorrhea, our state fails to acknowledge the serious impact it can have on their wellness and productivity. Furthermore, from an economic standpoint, New Jersey is missing out on millions of dollars in tax revenue due to the restrictive nature of its medical marijuana law. Spencer pointed out that menstrual cramps may have been unfairly left off the list because only half of the population suffers from them. Even among women, most do not experience the intense symptoms that hit sufferers of conditions like dysmenorrhea or endometriosis, said Spencer. For much of history, medical issues that impact women have been ignored or downplayed, leaving far too many individuals to suffer in silence. This legislation recognizes that, in severe cases, dysmenorrhea can be a debilitating condition that has a limiting effect on womens lives. So whether you need more techniques to make yourself appear more like what you want or if you want to improve your health, take a look at the tips in this article.
An exercise class is a great way of continuing your fitness routine through the winter months. Many people are less inclined to exercise during the winter, especially if they have an outdoor routine. Try signing up for something completely different to your regular exercise: Try yoga if you typically cycle. Try free weights if running or jogging is your preferred routine. If nothing else, it's a healthy way to get through the dark winter months, who knows, you may find that you really enjoy this new form of exercise, and! Set goals. Having something you're working toward makes the exercise worthwhile. Whether you want to fit into a dress, run a marathon or look great at a reunion, setting goals helps you to see a light at the end of the exercise tunnel and helps you to focus on that goal. Instead of only doing seated calf raises or only doing standing calf raises, mix it up! Calves are actually made up of two separate muscles. By doing both straight-leg reps and bent-leg reps, you are ensuring that both muscles in your calves are getting a proper workout. This will allow you to see noticeable results much sooner. Before you begin your exercise routine, warm up your brain. It is just as important to get your central nervous system ready for activity, as it is to prepare your muscles. By doing this you will get more efficient workout, your central nervous system tells your muscles when to contract and. As you prepare for a strength training workout, skip the pre-workout warmup on the treadmill. Instead, increase the efficiency of your workout by spending those fifteen minutes warming up the targeted muscle groups that you will be working on during your session. You can then spend those fifteen minutes on the treadmill, after you complete your routine. Not only should you do crunches, but you need to add in cardio, if you want a flat belly. Kickboxing is a great aerobic workout, which will help tone your midsection. These powerful kicks give your abdominal muscles a great workout. Tae kwon do is also a great cardio ab workout. Think like the tortoise and not necessarily the hare! For some people long periods of slow low-intensity exercise can be more beneficial than short bursts of activity. Take a long walk with your mate in the evenings, play a game of croquet, or show your stuff in a game of horse shoes. Do something that relieves your mind while slowly exercising your body. For maximum swim training, focus on increasing your ankle flexibility. Flexible ankles make for more fin-like movements underwater and therefore make you swim much faster. To help your flexibility, sit barefoot on the floor with your legs outstretched and your heels on the floor. Point your toes straight out then back again towards your shins. Do this for at least 1 minute a day. If you find an exercise that you do not enjoy doing, start doing it. A lot of times, the reason that you do not like or enjoy the exercise is that you are weak at it. If you begin doing the exercise regularly, your strength will increase in the muscles that you use, and you may find that you actually enjoy doing the exercise. Try increasing your foot speed in speed sports by doing some foot exercises. Start by placing your feet hip-width apart and placing your hands at your sides. Stretch one leg out in front of you and touch it with the opposite hand, then lower it back to the floor. Do the same with the other foot and hand, except do it from behind you. Do these for about 20 seconds. Try to pick-up speed doing them for maximum results. A great fitness tip is to measure your stats often. You should aim to measure waist size, body fat, or some other variable at least every four weeks. This will show you the results that your training has given you, which will motivate you even more to accomplish your goals. It is important to stretch for at least 60 seconds before a workout, for people over 40. The less flexible your muscles are, this is because the older a person is. For people 40 and under, it is recommended that you stretch for 30 seconds prior to a workout. Once you begin running long distances, remember that your shoes are like tires for a high-performance sports car, which means, they wear out fast! You should replace your running every 500 miles for best performance. If you are a serious runner and long before they fall apart, running shoes lose their cushioning ability, this is a serious concern. Using worn-out running shoes puts your joints at risk of injury. A great tip to help you get in shape is to start playing rugby. Although rugby can be a very tough sport, it can also get you in very good shape from all the running and strenuous activity. Rugby can be a great way to get fit if you have the stomach for it. Have a personal goal that you want to meet. Keep this in your head so you have something to work for daily. If you set a goal for yourself it will help you stay motivated to achieve it. Once you reach your goal you should reward yourself for it, then make a new goal to work towards. Don't stop exercising the opposite one if you injure one of your arms. Technically, when you work out one of your arms the muscle nerves in the opposite arm are stimulated too. It's been found that working out one arm can increase the strength in the other by ten percent. When training with weights, you should not use a weight lifting belt. Using a weight belt will, over time, gradually weaken your lower-back and abdominal muscles. Training without a belt will increase the strength of your back and ab muscles and force you to lift with proper form and technique. Aligning the spine properly to ensure good posture is more important than you may think. Improper postures can affect your rib cage, damage your lungs and heart and cause gastrointestinal problems. Your slouch is reversible as long as you do some slight changes. Examples to improve posture are by using a headset for your phone, centering your computer monitor and lowering your armrests on your office chair. Commit yourself to fitness. Find something that motivates you. It might be hard to find the time and the motivation to exercise, especially after a busy day. Make an active decision to change your lifestyle when you choose fitness. Do not let someone else convince you to do it fitness is a personal commitment. Fitness is about more than big muscles.
